29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Employees were exposed to fire and explosion hazards. In Building 2, near powered industrial truck operations and the loading dock, three compressed gas cylinders were stored on the floor north of the receiving office, outside of an approved cabinet or room, exposing them to damage.

29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Employees were exposed to struck-by hazards. Employees engaged in forklift operations within building two were exposed to struck-by hazards in that the employer did not receive prior written approval from the manufacturer for modifications or additions before use. Forklifts are being used within building two were equipped with fork extensions, which extended two and a half feet past the original fork without approval.
