(a) Specialty Chemicals & Systems, Inc. at 4940 Jackson St., Denver, CO 80216: On and before October 9, 2020, the employer did not ensure all forklift operators were trained in the operation of powered industrial trucks and that the operator's training consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace. This condition exposed employees to struck-by hazards when employees were required to operate forklifts while moving material without having been trained and evaluated.

Abatement Note: The employer shall verify that the required workplace assessment has been performed through a written certification that shall include the following: 1) Identifies the workplace evaluated; 2) The person certifying that the evaluation has been performed; 3) The date(s) that the hazard assessment was performed; and 4) Which identifies the document as a certification of hazard assessment.
