29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) In the mailroom behind PRISM scanner #2, on or about 11/25/2020, IT and power cords on the floor created a tripping hazard for employees walking and working behind the equipment.  b) In the Adjudication Area at the intersection of the Battery Park and I-26 aisle, on or about 11/24/2020, conduit for IT and power cords on the floor created a tripping hazard for employee walking and working in the area.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Power strips (relocatable power taps) were  not used in accordance with UL instructions.   a) In the QC / Mail Out Room at the corner cubicle on Row 1, on or about November 24, 2020, a  power strip that was used to power office equipment was plugged into a portable multi-receptacle interruptible power supply unit instead of a permanent receptacle,  resulting in an electrical hazard.  b) At the warehouse workstation, on or about November 25, 2020, employees used the microwave that was plugged into an orange extension cord that was plugged into a power strip, resulting in an electrical hazard.  c) Regional Training Office, on or about November 25, 2020, a power strip that was used to power office equipment was plugged into an extension cord instead of a permanent receptacle, resulting in an electrical hazard.  d) Asst. Director's Office, on or about November 25, 2020, a small refrigerator was plugged into a power strip, resulting in an electrical hazard.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   a) At the warehouse workstation, on or about November 25, 2020, employees used the microwave that was plugged into an orange extension cord that was plugged into a power strip (relocatable power tap), resulting in an electrical hazard.  b) Regional Training Office, on or about November 25, 2020, a power strip that was used to power office equipment was plugged into an extension cord instead of a permanent receptacle, resulting in an electrical hazard.  c) Fraud Program Manager Office, on or about November 25, 2020, two extension cords were used to power office equipment, resulting in an electrical hazard.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a) Adjudication Area on the Antebellum Row, on or about 11/24/2020, two 32 oz. spray bottles used as secondary containers of cleaner were not properly labeled in that one container was identified as "cleaner" without any additional information and the other container was not labeled.
