29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) roof top of building, 57 N Main St, Medford, NJ - Employees, without any visible means of fall protection, were in the process of reroofing the approximate 45' x 25' x 13' high roof, on or about 11/25/2020.

29 CFR 1926.1060(b):Retraining was not provided for each employee as necessary so that the employee maintained the understanding and knowledge acquired through compliance with this section: a) East side of building, 57 N Main St, Medford, NJ - Employees were improperly using two different extension ladders, one 20' Werner aluminum extension ladder and one 16' Werner aluminum extension ladder; neither had their pulley rope, the 16' extension ladder was setup against the top of the bay window bump out at an excessively shallow angle, worker on the 20' extension ladder was balanced on one foot leaning way over to his right with his left hand on the top of the bump out while hammering nails into sheathing, and construction debris was at the foot of both ladders, on or about 11/25/2020.
