29 CFR  1910.132(a):Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about November 24, 2020, Superior Trim did not ensure that protective clothing such as but not limited to, coveralls and foot covering,  was provided and utilized by employees working with the hazardous chemical ELASTOFLEX*24501T ISOCYANATE (corrosive, skin and respiratory sensitization).

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  a) On or about November 24, 2020, Superior Trim did not ensure that the relocatable power taps (power strip), used to power submental equipment on the roll coater machine,  was used in accordance with manufacturer's instructions. The relocatable power tap was not directly connected to a permanently installed branch circuit receptacle but was plugged into a flexible extension cord.

29 CFR  1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  As a substitute for the fixed wiring of a structure;  a) On or about November 24, 2020, Superior Trim did not ensure that fixed wiring was used to provide electricity to the equipment on the roll coater machine such as but not limited to, the resin heating elements. a flexible extension cord was used to power a  relocatable power tap (power strip) which in turn powered the equipment.

29 CFR  1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.   On or about November 24, 2020, Superior Trim did not ensure that employees working with the hazardous chemical ELASTOFLEX*24501T ISOCYANATE (corrosive, skin and respiratory sensitization) had received effective training on the hazards associated with the chemical, in that employees were unaware of the hazards associated with the use of the chemicals and were unable to identify the hazard labels on the containers.
