The abatement documentation for citation 2, items 1a and 1b, was due by 3/22/2021 and 4/22/2021, respectively.

The abatement certification for citation 2, items 1a and 1b, was due by 3/22/2021 and 4/22/2021, respectively.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Scandinavian Athletic Club (SAC), 438 Lake St, Shrewsbury MA: On 12/1/20, employees were exposed to falls from a height of approximately 16 feet to the ground while removing the existing asphalt roof on a commercial structure in preparation for installing a new roof, and fall protection was not used.  The employees were not protected by any fall protection system, and two employees fell from the roof to the ground.  New System Quality & Exteriors Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1467715, citation number 1, item number 1, and was affirmed as a final order on 7/9/20, with respect to a workplace located at Raynham Preserve, MA.  New System Quality & Exteriors Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1379158, citation number 1, item number 1, and was affirmed as a final order on 3/29/19, with respect to a workplace located at Lot 195 Hood Dr, Andover MA.

Two of the employees had no fall protection and were injured when they fell from the roof to the ground.
