29 CFR  1928.57(a)(6)(iii):Stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment, except where the machine must be running to be properly serviced or maintained, in which case the employer shall instruct employees as to all steps and procedures which are necessary to safely service or maintain the equipment:  On or about December 1, 2020, the employer had not instructed each employee engaged in cleaning the automatic rotating milking carousel to ensure equipment was stopped and disconnected from the power source before accessing the top of the carousel, exposing employees to caught in hazards.

29 CFR  1904.29(b)(1):What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year:   On or about December 1, 2020, the employer did not record the following workplace injury correctly on the OSHA Log for calendar year 2020:   a) On or about November 27, 2020, a milking parlor employee, while performing cleaning tasks, sustained multiple lacerations and fractures to the lower portion of the body and arm [case log #1102]. The description recorded as "pinched in rotary" does not adequately describe the neither the nature of the injuries nor the location of where the incident occurred, as required in Column F on the OSHA Form 300.

29 CFR  1910.1200(f)(6): Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either (Agriculture Reference 29 CFR  1928.21(a)(5): Hazard communication - 1910.1200.):   On or about December 1, 2020, the employer had not ensure that secondary containers of teat dips in the milking parlor were labeled with either: 1) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, 2) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.
