29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  At the jobsite, employees were exposed to fall hazards of more than 10 feet while working on a roof without using fall protection.   Bengino Frias Alvarez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1240927, citation number 1, item number 1, and was affirmed as a final order on July 31, 2017, with respect to a workplace located at 76414 Mount Hope Drive, Cypress, Texas, 77433.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At the jobsite, employees were exposed to fall hazards of approximately 10-feet 6-inches and greater when they used two aluminum extension ladders at the front of the house that were not set three feet above the roof landing.   b) At the jobsite, employees were exposed to fall hazards of approximately 25-feet and greater when they used two aluminum extension ladders at the rear left side of the house that were not set three feet above the roof landing.    Benigno Frias Alvarez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1240927, citation number 1, item number 2, and was affirmed as a final order on July 28, 2017, with respect to a workplace located at 76414 Mount Hope Drive, Cypress, Texas, 77433.
