29 CFR 1910.23(b)(8): The employer did not ensure ladders were used only for the purposes for which they were designed: (a) On or about December 1, 2020, at the worksite located at 1401 Perkins Avenue, Waukesha, WI, employees used an eight (8) foot step ladder to access a 12 foot upper landing surface of a dust collect unit to perform rigging work. This exposed employees to a 12 foot fall hazard to the lower level.

(a) On or about December 1, 2020, at the worksite located at 1401 Perkins Avenue, Waukesha, WI, employees used an eight (8) foot step ladder to access a 12 foot upper landing surface of a dust collect unit to perform rigging work. Employees used the top cap of the ladder and top step of the ladder as a step to access the upper landing surface of the dust collector. This exposed employees to a 12 foot fall hazard to the lower level.
