29 CFR  1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):    On or about December 2, 2020, the employer failed to develop and conduct training for employees on hazard recognition of the various chemicals they may encounter while performing their normal job duties to include but not limited to oils and lubricants.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about December 2, 2020, the employer failed to develop and implement a program to address the control of hazardous energy in order to protect employees from accidental start up or release of stored energy when performing tasks such as but not limited to clearing jams, changing knives, service and repair on machines such as but not limited to the 18 inch Yoder  Slitter, Serial #Y4335-165.  On or about December 2, 2020, the employer failed to develop and implement a program to address the control of hazardous energy in order to protect employees from accidental start up or release of stored energy when performing tasks such as but not limited to clearing jams, changing knives, service and repair on machines such as but not limited to the 18" Tishkin Slitter, Serial #13871.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about December 2, 2020, the employer failed to ensure that the 18" Yoder Slitter Machine, Serial #Y4335-165, was protected by some form of machine guarding in order to protect employees from exposure to hazards, such as but not limited to, stuck by, caught in between and at the point of operation.  On or about December 2, 2020, the employer failed to ensure that the 18" Tishkin Slitter Machine, Serial #13871 335-165, was protected by some form of machine guarding in order to protect employees from exposure to hazards, such as but not limited to, stuck by, caught in between and at the point of operation.

29 CFR  1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:   On or about December 2, 2020, the employer failed to conduct periodic inspections, at least annually, on the overhead gantry cranes and jib cranes, to include but not limited to, the de-ko 5 ton overhead crane.  These cranes are used daily by employees as part of their normal job responsibilities.  On or about December 2, 2020, the employer failed to conduct periodic inspections, at least annually, on the overhead gantry cranes and jib cranes, to include but not limited to, the de-ko 12 ton overhead crane.  These cranes are used daily by employees as part of their normal job responsibilities.  On or about December 2, 2020, the employer failed to conduct periodic inspections, at least annually, on the overhead gantry cranes and jib cranes, to include but not limited to, the Coffing 3 ton jib crane.  These cranes are used daily by employees as part of their normal job responsibilities.
