29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about December 2, 2020, employees were exposed to the hazard of falling siding, windows, tools, and debris when working below employees that were installing windows and siding.  The employees were not observed wearing any hard hats.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1926.452(k)(5): Scaffold platforms were bridged on to another.  a) On or about December 2, 2020, at the above addressed jobsite, employees were conducting window installation operations from a ladder jack scaffold, at fourteen (14') feet above the ground, that had two sections bridged together creating an uneven working surface, thereby exposing the employees to fall hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
