29 CFR 1904.39(a)(1):Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about November 24, 2020, an employee died of a work-related incident, which had occurred at a work site located at 1800 N Route Z, Columbia, MO. The employer did not report the work-related fatality to OSHA within 8 hours. The employer failed to notify OSHA of the fatality.
