29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structural integrity to support employees safely: a) 103 Ridgedale Avenue, Morristown, NJ Employee installing a metal roof was exposed to injuries from up to an 18 foot fall to the hardware store's floor below as the worker stepped on the drop ceiling holding ceiling tiles and he fell 8 feet after going through the ceiling which could not support him and landed on top of a pergola on the display floor. Violation occurred on or about 10/19/2000.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 103 Ridgedale Avenue, Morristown, NJ Employee was exposed to injuries from a fall as his personal fall arrest system which consisted of a harness, 50 ft. vertical lifeline, a short lanyard, rope grab and anchorage was rigged such that when the worker stepped on and then fell through the drop ceiling, he fell at least 8 feet or more into the retail hardware store and onto the top of the pergola on display in the store. Violation occurred on or about 10/19/2000.
