29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  (a) On or about December 02, 2020,  employees working on the roof of a two story structure, did not use eye protection when using pneumatic nail guns and pneumatic cutting tool. This exposed employees to eye foreign body/impalement hazard from nails and construction debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about December 02, 2020, employees conducting roofing work on a 6:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 17 feet three (3) inches.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about December 02, 2020, the twenty foot aluminum extension ladder erected on the North side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the landing edge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately ten (10) feet.  (b) On or about December 02, 2020, the twenty foot aluminum extension ladder erected on the North end of the West side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the landing edge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately  Seventeen 17 feet three (3) inches.  (c) On or about December 02, 2020, the twenty foot aluminum extension ladder erected on the South end of the West  side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the landing edge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately Seventeen  (17) feet  three (3) inches.  (d) On or about December 02, 2020, the twenty foot aluminum extension ladder erected on the South side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the landing edge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately Seventeen  17 feet  three(3) inches.
