In the Warehouse, located at 1100 N Nova Road in Daytona Beach, FL: On or about: December 02, 2020, the employer exposed employees to struck-by and crushed-by hazards, in that the employer assigned employees to operate a powered industrial truck (forklift), without first completing the training and evaluation, as required by the standard.
