29 CFR  1910.134(m)(2)(i):The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E):  Willard Healthcare, LLC dba Blossom Nursing and Rehabilitation located at 370 East Howard St., Willard, Ohio 44890:  On or about December 7, 2020, and at times thereafter, the employer did not ensure the fit test records  included all the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E).  The employer required employees to wear N95 respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents. The records established for the fit tests administered did not identify the type of fit test performed, the specific make, model, style and size of the respirators tested and the pass/ fail results for all employees.
