29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Mountain Side Medical Colorado, LLC dba Tecomet - Boulder at 6265 Gunbarrel Avenue, Boulder, CO 80301:  On and before December 7, 2020, the employer did not ensure that step by step procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in any activities covered by this section.  This condition exposed the employees to amputation and crushing hazards on machines including, but not limited to:   (1) Willemin W 408MT (2) Cincom L20E (3) Cincom L20X (4) Cincom M20/M32 (5) Schmidt Press.

29 CFR 1910.147(c)(7)(i):  The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees, and to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  (a) Mountain Side Medical Colorado, LLC dba Tecomet - Boulder at 6265 Gunbarrel Avenue, Boulder, CO 80301:  On and before December 7, 2020, the employer did not provide adequate training to ensure that employees acquired the knowledge and skills necessary for the safe application, usage and removal of energy control devices, including but not limited to the following machines or equipment:  (1) Willemin W 408MT (2) Cincom L20E (3) Cincom L20X (4) Cincom M20/M32  (5) Schmidt Press.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met:  (a) Mountain Side Medical Colorado, LLC dba: Tecomet - Boulder  at 6265 Gunbarrel Avenue, Boulder, CO 80301:  On and before December 7, 2020, the employer did not develop, implement and maintain a written hazard communication program that pertains to employee use of chemicals, such as oils, fuels, cleaners and various other chemicals. This condition exposes employees to use chemicals in an unsafe manner due to lack of knowledge about the chemicals.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s):  (a) Mountain Side Medical Colorado, LLC dba: Tecomet - Boulder  at 6265 Gunbarrel Avenue, Boulder, CO 80301:  On and before December 7, 2020, the employer did not ensure that safety data sheets were updated and/or readily available to employees during each work shift when they were in their work areas. Safety data sheets in the workplace were not present and were not readily accessible to employees. This condition exposed employees to skin irritation and respiratory illnesses associated with the handling of hazardous chemicals.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)   Mountain Side Medical Colorado, LLC dba Tecomet - Boulder  at 6265 Gunbarrel Avenue, Boulder, CO 80301:  On and before December 7, 2020, the employer did not establish and implement a written hazard communication program in accordance with this standard nor did the employer train employees on hazardous chemicals in the work area. This condition exposes employees to use chemicals in an unsafe manner due to lack of knowledge about the chemicals.
