29 CFR  1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.  a)  North side, near Northwest corner of house, 207 20th Street South, Brigantine: Employees erected and installed vinyl siding from a mixed component aluminum pump jack scaffold system with an approximate 36 foot high aluminum pole within approximately 10 feet of the 12.47 kV primary, on or about 12/05/2020.  b)  North side, near Northwest corner of house, 207 20th Street South, Brigantine:  Approximate 36 foot high western most Alum-A-Pole employees were trying to lay down,  contacted the approximate 30 foot high 12.47 kV primary, on or about 12/06/2020.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

A competent person did not direct and supervise the work, on or about 12/05/2020.
