29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about December 1, 2020, employees were exposed to machine hazards associated with moving parts and in-going nip points, while unguarded chains and sprockets and areas surrounding were cleaned during the normal production mode of operation. The servicing and cleaning was performed on the Line #9 Brown Thermoformer Model # SRS-3036S, and involved an employee cleaning the inside of scrap plastic by use of a pneumatic air hose in the area of unprotected chains and sprockets.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work  [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about December 1, 2020, the employer did not ensure periodic inspections of energy control procedures were conducted annually, including a review of each authorized employee's responsibilities under the energy control procedures.  Authorized employees are required to conduct and direct service/maintenance activities on the machines, requiring them to perform lockout/tagout.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  On or about December 1, 2020, and at times prior, the employer did not ensure that sprockets and chains were enclosed.  Employees were exposed to the hazard of being caught in the sprocket and chain on the Line #9 Brown Thermoformer. The sprocket and chain near the stacker was not enclosed.
