29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Federal-Mogul Powertrain LLC, on or about December 2nd, 2021, at the location of 150 Fisher Ave., Van Wert, OH, 45891: An employee was exposed to caught-in hazards while performing maintenance on a high volume oil conveyor that was energized. A guard was removed and the machine was energized and operating when the employee's left index finger was caught between a chain and sprocket.
