29 CFR 1903.19(c)(1): Employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:   Location:  69 East Middle Patent Road, Stamford, Connecticut  Abatement certification and verification for Citation 01, Item 001b with a final order date of 5/5/2021 was not received within 10 calendar days (by  5/2/2021) following the abatement due date (4/22/2021).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems.  Jobsite: An employee working on a barn re-roof with a 4-pitch roof, with an eave height of approximately 19 feet above the ground, was not protected from fall hazards by the use of guardrails, safety nets, or personal fall arrest systems.  On or about December 6, 2020 an employee, while replacing a piece of plywood on a barn roof, fell through the plywood roof approximately 18 feet to the ground.  The employee was taken to Stamford Hospital for serious injuries which included broken bones.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   Jobsite:  The employee who was working on a 4-pitch barn roof approximately 19 feet above ground level was not trained in fall hazards and the procedures to be followed to minimize such hazards.
