29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal arrest system. (a) On or about December 08, 2020, employees conducting roofing work on an 6:12 pitch commercial roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9) feet five (5) inches on the West and East sides of the structure.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) On or about December 08, 2020, the extension ladder erected on the west side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the edge, and/or was not secured to the structure when it was not extended three feet above the landing surface.
