29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (c)(4)(ii)(B)   Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  On or about 11/30/2020 the employer did not provide adequate machine specific lock out procedures that provided means to secure all energy sources prior to loaders and operators conducting tasks on the AIDA Straightener Feeder (model # LFG-600) such as setup and adjustments in that the procedures utilized were not lockout procedures that covered the equipment.  They were procedures that covered the  interlocked gate that employees had to enter to access the Feeder. The gate had safety procedures attached to them titled "Safety Plug Procedures".  By following the "Safety Plug Procedures" instead of lockout procedures, the pneumatic power to the coil guides and the coil positioning handle were not addressed resulting in employees being exposed to a struck by hazards.

29 CFR  1910.147(c)(7)(i):The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  (c)(7)(i)(A)  Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or about 11/30/2020 the employer did not ensure that authorized employees such as loaders and operators were properly trained to control the energy sources for the AIDA Straightener Feeder (model # LFG-600). Authorized employees performing adjustments and set up on the Feeder were exposed to struck by hazards due to having no knowledge that the procedures on the interlocked gate did not including the means of securing the pneumatics for the coil guides and the coil positioning handle.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (2) turn off and shut down the machine  (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (4) affix lockout or tagout devices to each energy isolating device  (5) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.  (6) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished.  On or about 11/30/2020 the employer failed to implement energy control application steps as the Straightener Feeder was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.

29 CFR  1910.147(d)(5)(i):Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.  On or about 11/30/2020, an operator working on the AIDA Straightener Feeder (model # LFG-600) noted that the coil on the feeder was not feeding properly, the operator entered an interlocked gate. The operator followed the Safety Plug Procedures instructions on the gate, pushed the coil back in place, as a result the pneumatic air controlling the coil guides and the coil positioning handle activated. This resulted in the employee being struck in the face by the coil handle.

29 CFR  1910.217(f)(2):Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed.  On or about 11/30/2020 the employer did not provide adequate training to ensure employees understood safe methods of operating the part revolution press AIDA 250 Power Press model # NCS-250(2) and its auxiliary parts such as the AIDA Straightener Feeder (model # LFG-600). Operators and loaders were trained utilizing an inadequate safety procedure that did not secure the pneumatic power to the straightener feeder coil when employees would enter the interlocked gate to setup and make adjustments to the coiler.
