At meat department, located at 4651 Old Pleasant Hill Road, Kissimmee, FL, on the Hobart meat cutting saw, model 6801, the guard was not adjusted to the height/thickness of the meat being cut, exposing employees to amputation hazards, on or about November 19, 2020.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. On or about November 19, 2020, at 4651 Old Pleasant Hill Road in Kissimmee, FL, an employee suffered a work-related amputation. The employer was aware of the work-related amputation on November 19, 2020 at 4:59 PM. The employer notified OSHA on November 24, 2020 at 5:30 PM of the work-related amputation.
