The employer did not have machine specific written procedures for locking out/tagging out the multi-energy machines to safe guard employees performing services and/or maintenance on machinery or equipment, the machines are equipped with electrical(hardwire) and hydraulic , on or about December 9, 2020. a. Stationary Bandsaw - 240V b. Stationary Cold Saw - 220V c. 70-Ton Iron Worker - 240V and Hydraulic

Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�� twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section: 2545B Monroe Drive, Gainesville, GA (5 Ton Overhead Cranes throughout the shop) - Employees were exposed to struck-by/overhead hazard. Employees were required to operate the overhead 5-ton gantry cranes. Frequent (daily/monthly) inspections were not conducted of the hoist and hooks, with a certification record, on or about December 9, 2020. a. 5-Ton Overhead Crane - Middle Bay Area b. 5-Ton Overhead Crane - Paint Shop Area
