29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  Roswell Street Baptist Church (Bldg. C) 774 Roswell Street, Marietta, GA 30060, Roof: On or about 12/08/2020, The employer exposed employees to fall hazards of up to 54-feet to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities on a 3/12 pitched roof.

C) 774 Roswell Street, Marietta, GA 30060, Roof: On or about 12/08/2020, The employer exposed employees to fall hazards of up to 54-feet to the lower level, in that fall protection training was not provided to employees as to recognize hazards related to falling and what procedures to follow to minimize these hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   Roswell Street Baptist Church (Bldg. C) 774 Roswell Street, Marietta, GA 30060, Roof: On or about 12/08/2020, The employer exposed employees to fall hazards of up to 25-feet while accessing the ladder to the roof or ground level, in that a ladder with side rails extending at least 3-feet was not provided to allow for safe access on and off the roof.

C) 774 Roswell Street, Marietta, GA 30060, Roof: On or about 12/08/2020, the employer allowed the forklift driver to operate a forklift truck on the jobsite without a forklift certification. The operator could not produce a certification or operator's license at the time of inspection.
