C) 774 Roswell Street, Marietta, GA 30060, Roof: On or about 12/08/2020, The employer exposed employees to fall hazards of up to 54-feet to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities on a 3/12 pitched roof.

C) 774 Roswell Street, Marietta, GA 30060, Roof: On or about 12/08/2020, the employer allowed the forklift driver to operate a forklift truck on the jobsite without a forklift certification. The operator could not produce a certification or operator's license at the time of inspection.
