29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide. On December 1, 2020, employees were conducting framing work on the exterior walls of a residential building under construction from wood job-made scaffold. The walking/working surfaces were not 18 inches wide exposing employees to fall hazards greater than 10 feet.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: On December 1, 2020, employees at this residential dwelling under construction utilized a wood, job-made scaffold without fall protection. Employees were exposed to a fall hazard greater than 10 feet.
