1910.134(k)(5)(i)  Retraining was not conducted when changes in the workplace or the type of respirator rendered previous training obsolete:  Masonic Health System of Massachusetts, Inc., DBA Overlook Masonic Health Center, Inc. Located at 88 Masonic Home Rd, Charlton MA 01507: On or about 11/13/20, employees engaged in personal care of residents were exposed to aerosolized SARS-CoV-2 virus and the employer did not retrain employees at the beginning of the outbreak in November 2020.  At a minimum, training should have included: �	Why the respirator is necessary (N95 NIOSH-certified filtering facepiece respirators were mandatory) and what compromises the fit �	Limitations and capabilities of the N95 NIOSH-certified filtering facepiece respirators �	How to use the N95 NIOSH-certified filtering facepiece respirators in an emergency, including situations where the respirator may malfunction �	How to inspect, put on and remove, use, and check the seals of the respirator �	Procedures for maintenance and storage of the N95 NIOSH-certified filtering face-piece respirators �	How to recognize medical signs and symptoms that may limit or prevent the effective use of the N95 NIOSH-certified filtering facepiece respirators
