29 CFR 1910.30(a)(3)(iv):29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. (a) On or about December 8, 2020, an employee was repairing a sign from a Wilkie Ladder Truck (Model #60 & Serial #1601539) and the employee was not properly tied off. The employees lanyard was not attached to the D-ring of the safety harness located between the shoulders. This exposed the employee to an approximate 15 foot fall hazard.
