Employees engaged in grain handling operations in the east elevator while using the grain tripper are exposed to caught-in and amputation hazards in that the sprocket wheels and chains that were less than 7 feet from the floor and platform were not enclosed.

29 CFR 1910.253(b)(2)(ii):Inside of buildings, cylinders were not stored in well-protected assigned locations to prevent knock over. Employees were exposed to fire and explosion hazards. Employees engaged in activities within the maintenance shop were exposed to fire and explosion hazards in that an oxygen and acetylene tank were not stored in a location to prevent being knocked over. These cylinders were stored together in the maintenance shop against the maintenance shop wall unsecured.
