The employer is failing to protect employees from struck by hazards.  The hazard was most recently documented on or about December 7, 2020 at Stouse, LLC, located at 300 New Century Pkwy,  New Century, KS 66031.  Employees working in the make ready section of the warehouse are exposed to stuck by hazards. The Doosan electric forklift  number  two is in need of repairs in the following instances:  a)  The fork lift had three levers to operate the lifting mechanisms of the fork truck. The levers  did not have directional markings. b) The back up light to alert pedestrians in close proximity when the forklift is near by  was not operational.

The hazard was most recently documented on or about December 7, 2020 at Stouse, LLC, located at 300 New Century Pkwy,  New Century, KS 66031.   Employees operating the ten paper cutting machines located throughout the facility are exposed to amputation hazards. The material clamp  is operated by a foot pedal control while the employees hands are in the danger zone, holding the paper, while the clamp descends onto the material to be cut.   Stouse, LLC, was previously cited for a violation of this occupational safety and health standard for machine guarding, which was contained in OSHA inspection number 1463618, citation number 1, item number 2 and was affirmed as a final order on September 5, 2019, with respect to a workplace located at 300 New Century Pkwy New Century KS 66031.
