29 CFR  1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:   (a)  On or about December 10, 2020 - at the above addressed jobsite - an employee was observed working on a flat roof installing the  framing of a Pump Jack Scaffolding system without the use of personal fall protection, including but not limited to guardrail systems, safety net systems, or personal fall arrest systems. The employee was exposed to fall hazards of approximately twenty five (25) feet.

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   (a)  On or about December 10, 2020 - at the above addressed jobsite - employees performing shingling operations along the side of the roof wall were observed working on a Pump Jack Scaffolding system that was not equipped with midrails and endrails to prevent employees from fall hazards. Thereby exposing employees to fall hazards of approximately twenty (20) feet.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   (a)  On or about December 10, 2020 - at the above addressed jobsite - an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  Dream Home Restoration Services, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1353302,  citation number 1, item number 1 and was affirmed as a final order on January 1, 2019, with respect to a workplace located at 11346 Abbey Road,  Mokena, IL 60448.
