29 CFR 1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: 549 Wilbraham Rd., Springfield, MA: On or about December 10, 2020, an employee was observed climbing up the ladder used to support the scaffold, then over the scaffold plank to reach his work location more than 18 feet above the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. 549 Wilbraham Rd., Springfield, MA: On or about December 10, 2020 employees were observed roofing this residence from a ladder jack scaffold without any fall protection more than 18 feet above the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
