Location: 44 North Gate Park, Newton, MA.  a) On or about December 8, 2020, An employee fell from the roof due to not being connected to his lifeline.  b) On or about December 8, 2020, Employees were exposed to fall hazard greater than sixteen feet above ground level from a residential roof where an incomplete personal fall protection was used.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Location: 44 North Gate Park, Newton, MA.  On or about December 8, 2020, four employees were not protected from falling while using two ladders that did not extend three feet above roof decking.
