29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: A) on or about December 10, 2020 on the job site at 41-51 Athol Street, Dayville, CT : The employer did not provide hard hats to employees working on the ground level collecting trash. Employees were exposed to bodily injury from falling objects such as an electric saw or air gun used by roofers 15 feet above.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): A) On or about December 10, 2020, at the 41-51 Athol Street, Dayville, CT job site: Employees were performing residential house roof replacement activities at this job site. There were three (3) employees on the roof of structures without any fall protection. The fall was approximately 15 feet to the ground below.
