(a) On or about December 9, 2020, a contractor employee was not trained to recognize the hazards of the overhead piping that was part of the ammonia refrigeration process, while the employee was working on a self-powered platform ( JLG Scissor Lift Model 2632ES Serial number 0200248875) to replace light fixtures located on the ceiling in the ammonia mechanical room.

Pictorial methods of instruction, may be used, in lieu of written work procedures, if employee communication is improved using this method. The operating manuals supplied by manufacturers for platform system components can serve as the basis for these procedures. (a) On or about December 9, 2020, Dole did not provide written work procedures for operation, safe use and inspection of the self-powered platform (JLG Scissor Lift Model 2632ES Serial number 0200248875) that was used to perform building maintenance in the ammonia mechanical room.
