29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: At jobsite located at 10908 Steeplechase Ct. Wichita, KS, the employer did not ensure that appropriate permission from the manufacturer was obtained by the user prior to using an attachment on the Lull Hi Lander II forklift, and did not ensure that markings for the capacity, operation and maintenance were changed accordingly. This condition exposes employees to struck-by and crushing hazards. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the jobsite located at 10908 Steeplechase Ct. Wichita, KS, employees engaged in residential framing activities, walking the top plates, were exposed to fall hazards of approximately 11 feet from the top plate of the upper level of the residence to the ground below of the structure in that fall protection was not utilized. Scot George Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1469823, citation number 1, item number 3 and was affirmed as a final order on 04/16/20, with respect to a workplace located at 2863 N Anna Ct Wichita, KS 67213. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
