29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: a) At 10558 Eastern Shore Drive, Bldg. 1600 The Arlington at Eastern Shore Apartments, Spanish Fort, Alabama: On or about November 30, 2020, and at times prior; JRC Incorporated, failed to protect subcontracted employees from Jonathan Restorations, Inc., from struck-by hazards in that the company built a job-made box used to transport supplies to the crew located on the roof and the box was not approved by or designed by the manufacturer of the Powered Industrial Truck (JLG Serial number: 0160046732).

29 CFR 1926.451(c)(2)(v):The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) At 10558 Eastern Shore Drive, Bldg. 1600 The Arlington at Eastern Shore Apartments, Spanish Fort, Alabama: On or about November 30, 2020, and at times prior; JRC Incorporated, failed to protect subcontracted employees from Jonathan Restoration, Inc. from struck-by hazards in that in that a job-made box used to transport supplies to the crew located on the roof was not secured to the Powered Industrial Truck (JLG Serial number: 0160046732) to prevent the movement of the box.
