29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. 29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about July 31, 2020 employees engaged in residential roofing activities were not protected from a fall, exposing employees to a fall of 18.5 feet.  MILLENNIUM GENERAL CONTRACTORS, CORP. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1432764, citation number 1, item number 2a and was affirmed as a final order on November 1, 2019, with respect to a workplace located at 1625 Center Ave. Chicago Heights, IL 60411.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 31, 2020 employees working at height were not trained to minimize or recognize fall hazards, exposing the employees to a fall.  MILLENNIUM GENERAL CONTRACTORS, CORP. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1432764, citation number 1, item number 2b and was affirmed as a final order on November 1, 2019, with respect to a workplace located at 1625 Center Ave. Chicago Heights, IL 60411.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
