29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) V-Folder: On or about November 29, 2020 and at times prior, employees operating the Fen-Tech Industries V-Folder are exposed to amputation hazards from in running nip points of the unguarded rollers on the Rewind Side of the machine. (b) Modern Pouch: On or about December 10, 2020 and at times prior, employees operating the Modern Manufacturing Pouch Machine are exposed to amputation hazards from in running nip points of the unguarded rollers on the Unwind Side of the machine. (c) Ro-An #2: On or about December 10, 2020 and at times prior, employees operate the Ro-An Industries Freedom Series Pouch Machine while being exposed amputation hazards from in running nip points of the unguarded rollers on the Unwind Side of the machine. Note: Abatement certification and documentation are required for this item. Yellowstone Plastics, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 12335561, citation number 1, item number 3a and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 3725 W. 65th Street South, Idaho Falls, ID 83402.
