29 CFR 1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a.) On or about December 14, 2020, employees were not using a fall restraint system while operating and working from an elevated extensible and articulating boom platform aerial lift and exposed to a fall hazard.

29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a.) On or about December 14, 2020 employees were exposed to a 15 foot 6 inch fall hazard while performing framing work on a flat, newly constructed commercial roof in that some employees were not using any form of convectional fall protection.
