29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) NASA Tunnel #2 Hatch near EJ-21: On or about 12/09/2020, employees were not protected from a possible fall in that the landing used for storing equipment had unprotected sides as a result employees were exposed to a 7.5ft fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) NASA Tunnel #2 Hatch near EJ-21: On or about 12/09/2020, employees used an extension ladder to ascend and descend the tunnel without extending the ladder 3 feet above the upper landing surface. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
