29 CFR 1910.23(b)(13): The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder:  On or about December 10, 2020, on the west exterior of a residence located at 5840 NW 26th Court in Boca Raton, Florida, an employee performing beehive removal work was exposed to a fall hazard of approximately 16 feet while carrying objects such as, but not limited to a 6-foot fiberglass ladder while climbing up or down an extension ladder.

29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   On or about December 10, 2020, on the roof of a residence located at 5840 NW 26th Court in Boca Raton, Florida, an employee performing beehive removal work was exposed to a fall hazard of approximately 20 feet, 9 inches when working on the roof of a residence while not being protected from falling to the lower level.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017:  On or about December 10, 2020, at 5840 NW 26th Court in Boca Raton, Florida, an employee was exposed to a fall hazard when working from the roof of a residence prior to being provided with training to recognize and avoid the hazards associated with working from a roof.
