29 CFR  1928.57(b)(2)(i):The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets, and idlers were not guarded.  In the Flaker Room, employees engaged in milling activities were exposed to laceration hazards in that belts and pulleys were not properly guarded in the following instances:  1) In the Flaker room, east side of Flaker #1.  2)  In the Flaker room, west side of Flaker #4.

29 CFR  1928.57(c)(2)(ii):All revolving shafts, including projections such as bolts, keys, or set screws, were not guarded:  In the flaker room, employees engaged in milling activities were exposed to a caught in hazard in that Flaker 4 had an exposed 1.5 in diameter shaft, which projected approximately 2.5 inches and was unguarded.

29 CFR  1928.57(c)(4)(ii)(B)(2): Where removal of a guard or access door exposed an employee to any component which continues to rotate after the power is disengaged, the employer did not provide, in the immediate area,  a readily visible or audible warning of rotation, a safety sign warning the employee to look and listen for evidence of rotation, and not remove the guard or access door until all components have stopped:   In the flaker room, employees engaged in milling activities, were exposed to amputation hazards in that there was no readily visible or audible warning of rotation.
