(d)  On or about December 10, 2020 - 2200 7th Avenue SW, Cullman, AL, employees were exposed to struck-by hazards when the employee's practical instruction did not insure that the employee understood how to stop the powered industrial truck before striking objects in an emergency and nor insure that the employee kept a clear view of the direction of travel.
