29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: (a) Jobsite located at 2225 E Lincoln Hwy, Coatesville, PA: Employees were exposed to struck-by and caught-in-between hazards such as flying chips and clothing caught in rotating parts as a result of the employer's failure to ensure the Hunter BL5050 bench lathe, serial number GWF 4757, was equipped with a guard sufficient to protect the operator from the rotating chuck and flying debris created during operation. Condition noted on or about the 6-month period of June - December 14, 2020.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease) a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: (a) Jobsite located at 2225 E Lincoln Hwy, Coatesville, PA: Employees were exposed to fire and explosion hazards in that one compressed gas cylinder of oxygen was stored with one compressed gas cylinder of acetylene inside the cylinder storage area of the shop garage. Condition noted on or about the 12-day period of December 2-14, 2020.
