29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Electro Controls, Inc. - Sidney, Ohio: On or about January 28, 2021, the employer did not develop and implement an energy control program consisting of training, procedures, and audits, to protect employees from hazardous mechanical and pneumatic energy sources.  Employees that performed set-up tasks such as, but not limited to, applicator changeovers on the crimping machines, were exposed to amputation and caught-in injuries while servicing the machines.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a. Electro Controls, Inc. - Sidney, Ohio: On or about December 7, 2020, the employer did not assure the pneumatic wire cutting machine, used to cut cable wire to length, was guarded to protect employees from amputation injuries created by the machine's cutting blade.    b. Electro Controls, Inc. - Sidney, Ohio: On or about January 29, 2021, employees that operated the crimping machines, such as but not limited to the #11 AMP machine (s/n 73212) were exposed to amputation injury in that the pinch point(s), created by the terminal applicator, was not adequately guarded.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.132(d)(2):  The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  a. Electro Controls, Inc. - Sidney, Ohio: On or about December 7, 2020, the required workplace hazard assessment did not include a documented hazard assessment with written certification that identified the person certifying the workplace evaluation and the date of the hazard assessment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
