29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a)  Pedro Martinez, at a jobsite located at 584 Ottertail Drive, Lot 1498, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020, the employer did not ensure employees wore safety glasses while utilizing power tools that presented struck-by hazards to the eyes and face. A framer was using a pneumatic nail gun/circular saw. The employee was not utilizing eye protection, thereby exposing the employee to struck-by hazards to the eyes.

29 CFR  1926.404(b)(1)(iii)(C):Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  a) Pedro Martinez, at a jobsite located at 584 Ottertail Drive, Lot 1498, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020, the employer did not ensure extension cords were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Roofers were using a yellow extension cord with missing insulation along its length and a missing ground prong, which exposed the employees to a 119 volt electrical contact hazard.  b) Pedro Martinez, at a jobsite located at 584 Ottertail Drive, Lot 1498, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020, the employer did not ensure extension cords were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Roofers were using a yellow extension cord with the ground prong missing from the male end of a cord, which exposed the employees to a 119 volt electrical contact hazard.  b) Pedro Martinez, at a jobsite located at 584 Ottertail Drive, Lot 1498, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020,, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. 29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a) Pedro Martinez, at a jobsite located at 584 Ottertail Drive, Lot 1498, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020, the employer did not ensure employees were adequately protected from falls while engaged in residential construction activities. An employee was installing roof trusses without being connected to a fall protection system, thereby exposing them to fall hazards of up to 12 feet.  b)  Pedro Martinez, at a jobsite located at 584 Ottertail Drive, Lot 1498, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020,, the employer did not ensure employees were adequately protected from falls while engaged in residential construction activities. Employees were exposed to an approximate fall hazard of nine feet from an unprotected four foot wide opening to the basement from the first floor.

An employee was performing roofing work with the webbing of a retractable personal fall protection device tied directly around the top of the trusses at the truss nail plate, thereby exposing the employee to fall hazards of up to 12 feet.
