29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about December 12, 2020, and at times prior thereto, two roofers installing new sheet metal roofing did so while near unguarded skylights that were on an existing building, exposing the roofers to a fall hazard of approximately 25 feet to the concrete slab or transformers staged on pallets below.

29 CFR  1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval.  On or about December 12, 2020, and at times prior thereto, a JLG Telehandler Rough Terrain Forklift was used to move steel rafters into place while rigged with a single web strap to a single forklift tine without the prior written approval of the manufacturer to lift loads in such a manner, exposing the ironworkers receiving and welding the rafters into place to the hazard of being struck by the steel rafters.
