29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: a) On or about December 17, 2020, on the north face of the building, employees were standing on a 2x6 plank between scaffolding levels at a height of approximately 17 feet while attempting to reach a higher work area to apply stucco.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) On or about December 17, 2020, on the north face of the building employees were observed applying stucco while utilizing tube type scaffolding , that did not have a proper means of accessing or egressing.
